In short

Denecimig (also known as Mim8) is an investigational bispecific antibody being studied for people with haemophilia A, with or without factor VIII inhibitors. It is designed for routine prophylaxis to help the body form blood clots. The EU Clinical Trials Information System lists two registered trials for denecimig, both in phase 3, together enrolling 184 participants.

Key points

  • Denecimig is an investigational bispecific antibody that mimics factor VIIIa, designed for preventive treatment of haemophilia A.
  • Two phase 3 trials are registered: FRONTIER 4 (124 participants) and FRONTIER 5 (60 participants).
  • Both trials study people with haemophilia A with or without factor VIII inhibitors.
  • FRONTIER 4 is authorised and investigates long-term safety; FRONTIER 5 is completed and studied switching from emicizumab to denecimig.
  • Denecimig is given as a solution for injection under the skin (subcutaneously).
  • Published FRONTIER2 data showed denecimig significantly reduced annualised bleeding rate compared to prior prophylaxis and on-demand treatment.

What is denecimig?

Denecimig (development name Mim8) is an investigational bispecific antibody that acts as a factor VIIIa (FVIIIa) mimetic. It is a protein-based substance produced by Novo Nordisk A/S and is given as a solution for injection under the skin. Denecimig has no marketing authorisation in the product records, meaning it is still an investigational medicine. The substance is registered in the EU Clinical Trials Information System under the name denecimig and appears as the test substance in four trial records.

How does denecimig work?

Denecimig is designed to help the body form blood clots by mimicking the action of factor VIIIa, a protein that is missing or does not work properly in people with haemophilia A. As a bispecific antibody, it binds to two different targets at once, bringing together the clotting factors that are needed to form a stable clot. This mechanism is intended to work regardless of whether a person has developed factor VIII inhibitors, which are antibodies that neutralise standard factor VIII replacement treatments.

Which conditions is denecimig studied for?

Denecimig is being studied for one main condition:

  • Haemophilia A — a rare, inherited bleeding disorder caused by a deficiency of clotting factor VIII. People with haemophilia A bleed for longer than usual because their blood does not clot properly. The trials include people both with and without factor VIII inhibitors, which are antibodies that can make standard factor VIII replacement less effective.

The FRONTIER 4 trial also includes an infant arm in some European countries for previously untreated or minimally treated children under 1 year of age with severe haemophilia A.

How is denecimig given?

Denecimig is given as a solution for injection administered subcutaneously, meaning under the skin. The published FRONTIER2 study evaluated dosing once monthly or once weekly in adults and adolescents aged 12 years and older.

What do the clinical trials show?

Two phase 3 trials for denecimig are registered in the EU Clinical Trials Information System. Both are interventional studies in people with haemophilia A, with or without factor VIII inhibitors.

The FRONTIER 4 trial (NCT05685238) is authorised and plans to enrol 124 participants. It investigates the long-term safety of denecimig prophylaxis. In France, Germany, Italy, the Netherlands, Poland and Spain, the study includes an infant arm for previously untreated or minimally treated patients under 1 year of age with severe haemophilia A.

The FRONTIER 5 trial (2022-502450-13-00) is completed and enrolled 60 participants. It evaluated the safety of denecimig prophylaxis during emicizumab washout in adults and adolescents who switched directly from prophylaxis with emicizumab. The primary outcome was the number of treatment-emergent adverse events.

Published 26-week results from the phase 3 FRONTIER2 trial, reported in the New England Journal of Medicine, showed that denecimig significantly reduced the annualised bleeding rate compared to prior clotting factor prophylaxis and on-demand treatment in people with haemophilia A, with or without inhibitors.

What side effects are known?

The FRONTIER 5 trial measured the number of treatment-emergent adverse events as its primary outcome, but the registry record does not report the specific events observed. The published FRONTIER2 results report safety data from 26 weeks of treatment, but the available source does not list individual side effects. Detailed safety information from the completed trials has not been published in the sources provided here.
