Ongoing Clinical Trials for Hypothyroidism

There are currently 2 ongoing clinical trials investigating new approaches to treating hypothyroidism and its related symptoms. These studies focus on combination hormone therapies and addressing specific complications like persistent fatigue and erectile dysfunction in patients with underactive thyroid conditions.

Clinical trial locations

Study on Levothyroxine and Liothyronine for Patients with Autoimmune Hypothyroidism and Persistent Tiredness

This trial is taking place in the Netherlands and investigates whether combining two thyroid hormones can better address persistent tiredness in patients with autoimmune hypothyroidism.

Who can participate: The study is looking for adults aged 18 and over who have been diagnosed with either clear or mild forms of the condition and have been taking levothyroxine treatment for at least six months. Participants must be taking a daily dose between 75 and 225 micrograms, with their thyroid-stimulating hormone levels in the normal range for at least three months. Most importantly, participants must experience severe tiredness that significantly affects their daily life for at least six months, despite current treatment. Participants need to speak and read Dutch fluently.

Who cannot participate: The study excludes patients who do not have autoimmune hypothyroidism, those who are not experiencing persistent tiredness despite levothyroxine treatment, individuals outside the specified age range, and people considered part of vulnerable populations who might need special protection.

What the trial involves: The study compares combination therapy using levothyroxine and liothyronine against levothyroxine alone. Levothyroxine is a synthetic form of the thyroid hormone thyroxine, while liothyronine is a synthetic form of triiodothyronine, which works more quickly in the body. Researchers want to find out if adding liothyronine to standard treatment can reduce tiredness more effectively. The trial lasts one year, during which participants take their assigned medication orally as tablets once daily. Regular follow-up appointments include physical examinations, blood tests, and questionnaires to assess quality of life and symptom changes. The study also explores whether genetic variations affect treatment outcomes.

Investigational drugs: The trial uses levothyroxine (LT4), which helps regulate the body’s energy and metabolism, and liothyronine (LT3), a different type of thyroid hormone that acts more quickly. Both are administered as oral tablets.

Study on Levothyroxine and Sildenafil for Improving Erectile Function in Patients with Subclinical Hypothyroidism and Erectile Dysfunction

This trial is being conducted in Spain and examines how thyroid hormone treatment affects sexual function in men with mild thyroid problems and erectile dysfunction.

Who can participate: The study is recruiting heterosexual males over 18 years old who are sexually active and have been diagnosed with subclinical hypothyroidism, meaning their thyroid-stimulating hormone level is greater than 5 with normal T4 levels. Participants must also have erectile dysfunction confirmed by scoring less than 25 points on a specific questionnaire called the International Index of Erectile Function. Participants must be able to complete several questionnaires measuring different aspects of sexual health and satisfaction, and must sign an informed consent form agreeing to participate.

Who cannot participate: The study excludes patients without subclinical hypothyroidism, those without erectile dysfunction, women, individuals outside the specified age range, and those belonging to vulnerable populations requiring special protection or care.

What the trial involves: The study assesses how levothyroxine, when adjusted for a patient’s weight and baseline thyroid hormone levels, impacts sexual activity in men with both conditions. Participants are randomly assigned to receive either levothyroxine, sildenafil (commonly used to treat erectile dysfunction by increasing blood flow), or a placebo. The study lasts three months, during which participants take their assigned medication orally. Throughout the trial, participants complete questionnaires to evaluate changes in erectile function and overall treatment satisfaction. Monitoring occurs at the beginning and after three months to track changes in their condition.

Investigational drugs: The trial uses levothyroxine to regulate thyroid hormone levels and improve various bodily functions including sexual activity, and sildenafil to compare its effects on erectile function with those of levothyroxine in patients with the condition.

Summary

These two clinical trials represent different approaches to addressing complications of thyroid problems. The Netherlands study focuses specifically on persistent fatigue in patients with autoimmune forms of the condition, investigating whether combination hormone therapy offers advantages over standard single-hormone treatment. The Spain study takes a unique approach by examining the connection between mild thyroid dysfunction and sexual health in men, comparing thyroid hormone treatment with conventional erectile dysfunction medication.

Both studies highlight that even when thyroid hormone levels are controlled with standard treatment, patients may continue to experience significant quality-of-life issues. The trials use levothyroxine as a key investigational drug, reflecting its established role in thyroid hormone replacement therapy. These studies may provide important insights into optimizing treatment strategies for patients who remain symptomatic despite conventional therapy.
