Metastatic brain cancer, also called brain metastases, happens when cancer cells from tumors in other parts of the body travel to the brain and form new growths. Unlike primary brain cancer that originates in the brain itself, metastatic brain tumors are far more common and represent a serious complication that affects thousands of people living with cancer each year.
Understanding Brain Metastases
Brain metastases occur when malignant (cancerous) cells break away from a tumor somewhere else in your body and spread to your brain through your bloodstream. The original cancer is called the primary tumor, and when it spreads to create new tumors in the brain, these are known as metastases or secondary brain tumors.[1][2]
Once cancer cells reach the brain, they can form one or more new tumors. These growths put pressure on the surrounding brain tissue, which can interfere with how your brain normally works. The effects you experience depend largely on where in the brain these tumors develop and how large they become. As they grow, they compress the delicate structures around them, leading to various symptoms and complications.[1]
The most common location for brain metastases is the cerebrum, which is the largest part of the brain at the top of your skull. Less frequently, cancer spreads to the cerebellum (the area at the back of the brain that controls balance) or the brain stem (which connects the brain to the spinal cord). Sometimes a person develops just one metastatic tumor, but more often, multiple tumors form throughout the brain.[3]
How Common Are Metastatic Brain Tumors?
Brain metastases are the most common type of brain tumor in adults. They are far more frequent than cancers that start in the brain itself. Most tumors that begin in the brain are actually not cancerous at all. In contrast, metastatic brain tumors always represent cancer that has spread from elsewhere.[2]
Experts estimate that between 10% and 30% of adults whose cancer starts outside the brain will eventually develop brain metastases at some point during their illness. In the United States alone, somewhere between 98,000 and 170,000 new cases are diagnosed each year. The number of people developing brain metastases appears to be increasing, and there are several reasons for this trend.[2][4]
First, newer cancer treatments are helping people with various cancers live longer. While this is good news, it also means the cancer has more time to potentially spread to the brain. Second, improvements in imaging technology, particularly magnetic resonance imaging (MRI), allow doctors to detect even small brain tumors that might not be causing symptoms yet. This leads to earlier diagnosis and a higher recorded incidence of the condition.[4]
The chances of being diagnosed with brain metastases increase as people get older, particularly after age 45. Most people who are diagnosed are over 65 years old. This age pattern reflects both the fact that cancer itself becomes more common with age and that it takes time for cancer to spread from one part of the body to another.[2]
What Causes Brain Metastases?
Brain metastases develop through a multi-step process. Cancer cells from a tumor in another part of your body break off from the main growth. These rogue cells then enter your bloodstream and travel throughout your body. When they reach the brain, they pass through the walls of blood vessels and enter brain tissue. Once inside the brain, these cells begin to multiply, forming new tumors.[2][4]
Scientists don’t fully understand why some primary cancers are more likely to spread to the brain than others. However, certain types of cancer have a particularly strong tendency to metastasize to the brain. The location and characteristics of the original tumor seem to play important roles in determining whether brain metastases will develop.[2]
The metastatic cells typically spread throughout the brain in areas with high blood flow. Different types of cancer may favor different locations within the brain. This distribution pattern affects which symptoms a person will experience, since different brain regions control different functions.[4]
Which Cancers Most Often Spread to the Brain?
While virtually any cancer can spread to the brain, some are much more likely to do so than others. Lung cancer is the leading source of brain metastases, accounting for about half of all cases. Among lung cancer types, small cell lung cancer has an especially high tendency to spread to the brain. Because of this strong propensity, people with small cell lung cancer are sometimes given preventive brain radiation even before any metastases appear.[2][3][5]
Breast cancer is the second most common source of brain metastases. Approximately 10% to 15% of people with metastatic breast cancer develop tumors in the brain. Brain metastases are particularly common in certain subtypes of breast cancer, specifically HER2-positive breast cancer and triple-negative breast cancer. In fact, in some people successfully treated for breast cancer with drugs like trastuzumab (which cannot cross into the brain effectively), the brain becomes the first site where cancer reappears.[2][6]
Melanoma, a serious form of skin cancer, also frequently spreads to the brain. About half of people with melanoma eventually develop brain metastases. Despite being less common than lung or breast cancer, melanoma’s high rate of brain spread makes it the third leading cause of metastatic brain tumors.[2][3]
Other cancers that can spread to the brain include kidney cancer, colorectal (colon) cancer, thyroid cancer, and prostate cancer. However, these occur less frequently. Some cancers, such as head and neck cancers, rarely result in brain metastases.[1][4]
Symptoms of Brain Metastases
The symptoms of brain metastases vary considerably from person to person. What you experience depends on several factors: where in the brain the tumors are located, how large they are, how many there are, and how quickly they’re growing. Because different parts of the brain control different functions, a tumor in one location will cause different symptoms than a tumor elsewhere.[1][2]
Headache is the most common symptom of brain metastases and often the first sign that something is wrong. These headaches occur because the growing tumor puts pressure on brain tissue, or because fluid builds up around the tumor causing swelling (called edema). Sometimes the headaches are accompanied by nausea and vomiting, particularly in children. Unlike typical headaches, these may be more severe in the morning or worsen with coughing, sneezing, or bending over.[2][3]
Seizures (also called fits or convulsions) are another common symptom, occurring in about 35% of people with brain metastases at some point. A seizure happens when abnormal electrical activity in the brain causes involuntary movements, changes in consciousness, or unusual sensations. For some people, a seizure is the very first indication that something is wrong with their brain.[2][5]
Many people experience weakness or numbness on one side of their body. This is called hemiparesis. You might have trouble using your arm or leg on one side, or you might lose feeling in your face, arm, or leg. These symptoms occur on the side of the body opposite to where the brain tumor is located. For example, a tumor on the right side of the brain causes weakness on the left side of the body.[2][3]
Cognitive problems are common and can include short-term memory loss, confusion, difficulty concentrating, and changes in personality or mood. Some people become more irritable or experience behavioral changes that concern their family members. These mental changes happen because the tumor interferes with the brain’s normal processing of information and emotions.[1][2]
Vision problems can develop if tumors affect the parts of the brain that process visual information or control eye movements. You might experience blurred vision, double vision, or loss of vision in part of your visual field, typically on one side or the other.[2][3]
Difficulty with speech is another possible symptom. You might have trouble finding the right words, understanding what others are saying to you, or speaking clearly. Problems with balance and coordination can make you feel unsteady on your feet or clumsy. Some people have difficulty swallowing or experience hearing problems.[1][2]
As brain metastases advance, additional symptoms may appear. Drowsiness is the most common symptom in end-stage brain metastases. Some people may lose control of their bladder or bowels (called incontinence).[2][3]
It’s important to understand that most brain metastasis symptoms develop slowly and worsen gradually over time. This is different from a stroke, where symptoms come on suddenly. However, in rare cases, brain metastases can cause a stroke-like event.[2]
Risk Factors
The primary risk factor for developing brain metastases is having cancer that originates in another part of your body. However, not everyone with cancer will develop brain metastases. Certain factors increase the likelihood that cancer will spread to the brain.[2]
The type of primary cancer matters greatly. If you have lung cancer, breast cancer, or melanoma, you face a higher risk than if you have other types of cancer. Within these categories, specific subtypes carry even greater risk. For example, people with HER2-positive breast cancer or triple-negative breast cancer are more likely to develop brain metastases than those with other breast cancer types.[2][6]
Age plays a role in risk, with chances increasing after age 45. Most diagnoses occur in people over 65. This partly reflects the fact that cancer itself becomes more common with age.[2]
If your cancer has already spread to other organs in your body, you’re more likely to eventually develop brain metastases as well. Cancer that is more advanced or aggressive also carries higher risk. Additionally, people whose cancer has certain genetic characteristics or molecular features may face increased risk, though scientists are still working to understand these relationships fully.[6]
How Changes Occur in the Brain
When cancer cells reach the brain, they must first break through the blood-brain barrier, a protective layer that normally keeps harmful substances out of brain tissue. Once cancer cells pass through this barrier, they begin to multiply and form clusters. These growing masses of cancer cells are what we call metastatic tumors.[4]
As these tumors grow, they cause several types of changes in the brain. First, they physically push against surrounding brain tissue, displacing normal structures and disrupting their function. Second, they trigger inflammation, the body’s natural response to abnormal cells. Third, they cause fluid to accumulate around them, creating swelling that further increases pressure inside the skull.[4]
Your skull is made of hard bone with a fixed amount of space inside. When tumors and swelling develop, there’s nowhere for this extra mass to go. This creates what doctors call increased intracranial pressure. The pressure can damage delicate brain structures and interfere with blood flow to important brain areas. Many symptoms of brain metastases, including headaches, vomiting, and changes in consciousness, result directly from this increased pressure.[3]
The tumors can also interfere with the brain’s electrical signaling. The brain works by sending electrical impulses between nerve cells. When cancer disrupts these signals, it can cause seizures, which are episodes of abnormal electrical activity. Tumors can also block the normal flow of cerebrospinal fluid, the liquid that cushions and nourishes the brain. This blockage causes a condition called hydrocephalus, which further increases pressure inside the skull.[3]
Each region of the brain has specialized functions. The tumors’ specific locations determine which functions are affected. A tumor near the front of the brain might cause personality changes or difficulty with decision-making. A tumor toward the back might affect vision or balance. Tumors on one side of the brain typically cause symptoms on the opposite side of the body because nerve pathways cross over as they travel from the brain to the body.[3]
Prevention and Early Detection
There is no guaranteed way to prevent brain metastases if you have cancer, but certain approaches may help reduce risk or catch problems early. For people with cancer types that frequently spread to the brain, healthcare providers sometimes recommend screening even before symptoms appear. Regular brain imaging with MRI can detect small tumors before they grow large enough to cause problems.[2]
For certain cancers, particularly small cell lung cancer, preventive treatment may be offered. This involves giving radiation therapy to the brain before any metastases appear, a technique called prophylactic cranial irradiation. This approach has been shown to reduce the risk of developing brain metastases in people with this type of cancer.[4][7]
If you have cancer that could potentially spread to the brain, maintaining close communication with your healthcare team is essential. Report any new neurological symptoms promptly, including persistent headaches, vision changes, balance problems, confusion, or seizures. Early detection of brain metastases generally allows for more treatment options and better outcomes.[1]
Managing your primary cancer effectively may also help reduce the risk of spread. Following your treatment plan, attending all scheduled appointments, and having recommended follow-up scans are important steps. Some newer systemic therapies, including certain targeted drugs and immunotherapies, show promise in preventing or controlling brain metastases, though research in this area is ongoing.[6]
Living a healthy lifestyle, while not proven to prevent brain metastases specifically, supports your overall health and ability to tolerate cancer treatments. This includes eating a balanced diet, staying as physically active as your condition allows, getting adequate rest, managing stress, and avoiding tobacco and excessive alcohol.[8]
