The research focuses on individuals living with Atherosclerotic Cardiovascular Disease, a condition where plaque builds up in the arteries, and or Chronic Kidney Disease, which is a long-term condition where the kidneys do not work as well as they should. Participants will be given either orforglipron, which is an oral tablet, or a placebo.

During the study, participants will be monitored over a period of time to see how the medication affects the occurrence of major health events. These events include myocardial infarction, commonly known as a heart attack, stroke, or being hospitalized due to heart failure. The study also looks at coronary revascularization, which is a procedure used to improve blood flow to the heart, and any deaths caused by any reason.

The study will evaluate the effectiveness and safety of a medication called gliclazide MR in patients whose diabetes is not well controlled with another medication, dapagliflozin, with or without the addition of metformin. Gliclazide MR is a modified-release tablet, which means it releases the medication slowly over time. Dapagliflozin and metformin are also used to help control blood sugar levels in people with Type 2 Diabetes.

The purpose of this study is to see if the combination of gliclazide MR and dapagliflozin, with or without metformin, is better at reducing blood sugar levels compared to a placebo and dapagliflozin, with or without metformin. Participants in the study will be randomly assigned to receive either the combination of medications or a placebo.
