Study on Dexamethasone for Patients with Herpes Simplex Virus Encephalitis

This clinical trial is examining whether dexamethasone, a corticosteroid medication, can improve recovery and brain function in patients with herpes simplex virus encephalitis, a serious brain infection. The study aims to determine if adding this anti-inflammatory medication to the standard antiviral treatment leads to better outcomes in memory and overall brain health six months after treatment begins.

Who can participate:

  • Adults aged 18 years or older
  • Patients with suspected encephalitis causing changes in consciousness, thinking, personality, or behavior lasting more than 24 hours
  • Laboratory-confirmed herpes simplex virus infection through a PCR test on cerebrospinal fluid (the fluid surrounding the brain and spinal cord)
  • Currently receiving aciclovir (an antiviral medication) through a vein at 10 mg per kilogram of body weight three times daily, or at an adjusted dose if kidney function is reduced
  • Must be affiliated with social security
  • Written informed consent provided by the patient or their legal representative

Who cannot participate:

  • Children between 3 and 4 years of age
  • Individuals considered part of a vulnerable population who may be at higher risk of harm or exploitation

What the trial involves:

Participants will receive either dexamethasone or a placebo (an inactive substance) in addition to their standard aciclovir treatment. The dexamethasone is given as an oral solution at a dose of 20mg/5ml. The medication works by reducing inflammation in the brain and modulating the immune system’s response, which may help decrease swelling and pressure.

Throughout the study, patients will undergo regular assessments to monitor their recovery. Memory and cognitive abilities will be evaluated using standardized tests like the Wechsler Memory Scale at various time points: 30 days, 6 months, and 18 months after starting treatment. These evaluations measure memory, processing speed, and other thinking abilities.

The study also monitors other important health outcomes, including the occurrence of seizures, length of hospital stay, overall survival, and quality of life. Safety is tracked by checking for the presence of the virus in spinal fluid and monitoring other health indicators. Researchers will also assess anxiety, depression, and overall health status using standardized questionnaires at 6 and 18 months.

Investigational drug:

Dexamethasone is a corticosteroid medication that reduces inflammation and is being tested to see if it can improve long-term brain function outcomes in patients with this condition.
